We're here to help you to live independently in your own home and stay connected to your community. Home care services are available through the Commonwealth Home Support Programme (CHSP) or Support at Home (SaH).
Once you have been assessed for aged care services by My Aged Care and determined your eligibility, we can provide you with a wide range of services, including:
- Home Care services
- Personal Care services
- Gardening and Home Maintenance services
- Access to Social Centres in and around Perth.

Personal Care
We're here to assist you to look and feel your best. We provide assistance with showering, dressing, grooming, oral hygiene and medication.
Gardening and Home Maintenance
There's no need to worry about your garden or those odd jobs around the house when you've got the support of Rise's friendly and trained gardeners.
Respite Care
We provide short-term respite and personalised care for guests who are ageing or living with dementia. Cottage respite is available at our picturesque country cottage in the Perth hills.
Social Centres
Stay active, make new friends and connect with your community. You'll find a whole range of activities and clubs at our social centres across Perth.
Arts Hub
Meet new people, learn new skills and have a whole lot of fun with painting, woodwork, pottery and craft.
Moorditj Aboriginal Social Group
Aboriginal and Torres Strait Islanders can get together at our friendly informal social group in Armadale.
Pricing
Many of our services can be provided under funding packages offered by the Australian Government so there is no or little cost to you.

You may be eligible for services if you’re:
65 years or older (50 years or older for Aboriginal or Torres Strait Islander people) and:
- Noticed a change in the things you do or what you can remember.
- Have a medical condition.
- Have reduced mobility and find it harder to get around.
- Had changes in your family care arrangements.
- Had a recent fall or been admitted to hospital?
The government-funded age care service is based on your current health, how you manage at home and the services you already receive.

The Commonwealth Home Support Programme (CHSP) is for people who just need assistance with a few tasks around the home.
